Coordinated Outreach is a web-based, community driven outreach management application that’s designed
to help outreach organizations maximize their efforts and leverage the resources of the community.
Key Features:
Recipient Tracking allows organizations to store information on individual recipients and track what resources have and have not been provided. This helps organizations reduce fraud and enforce policies.
Referral Functionality helps organizations leverage the resources in the community. When an individual is in need of assistance that an organization can’t provide or a resource that’s been depleated, organizations use the referral service to find nearby organizations that provide the service or resource that the individual
needs.
Comprehensive Reporting gives organizations the ability to report on their outreach efforts. The Coordinated Care application has several built-in reports and we’re expanding the library often. If our canned reports don’t suit your needs, you can export your information for use in Excel, Open Office, Crystal Reports, or any other reporting tool.
Pre-Built Forms can be built for any organization. For a small professional services fee, we can convert any form into a digital format that can be auto-filled by the Coordinated Care application. This allows organizations to go paperless where possible and still maintain governmnet compliance.
